Charges
Hokianga Health Enterprise Trust aims to provide services at no cost at the point of need.
Under the Primary Health Organisation service contracts introduced in 2004, the Trust receives Government funding to provide GP services to people who
have chosen to register with the Hauora Hokianga Integrated PHO. However, if a person is not registered with the PHO,
the Trust's policy is to charge a fee for GP & nursing services provided to people aged over 6 years old. These fees are subsidised if the person has a Community
Services or a Higher User Health Card. Please refer to the table below for the list of charges.
If you wish to enrol with the Hauora Hokianga Integrated PHO, please contact us on (09)4057709,
fill in an enrolment form at you local clinic, or print out and complete the details on this
form and fax to (09)4057875. You will be entitled to receive GP services at no charge immediately after presenting a completed enrolment form.
Please note that the adult dental services are limited to Hokianga residents who have lived in the area for at least the last three months.
This rule is independent on whether the resident is registered for the PHO or not. Unfortunatley the adult dental service is temporailiry suspended due to the unavailability of a dentist.

Nurse only clinics are half the listed price.
Please bring cash to the clinic to pay your fee, koha, or donation. An EFTpos facility is available at the Rawene Health Centre only.
