Primary Health Care Services
Community‑based primary care

Our Primary Health Care services support the health and wellbeing of individuals and whānau across all stages of life. This includes the diagnosis, treatment, and ongoing management of health conditions, as well as health education, disease prevention, screening, and referrals to hospital and specialist services when needed.

Hauora Hokianga provides Primary Health Care services from ten locations throughout the Hokianga. Our clinics offer comprehensive, integrated care and are closely connected with our wider support services.

Care is delivered by a dedicated multidisciplinary team, which may include Medical Doctors or Nurse Practitioners, Community Nurses, Kai Manaaki Tangata, and Clerical staff. Together, we work to ensure care is accessible, coordinated, and centred on the needs of our community.

All Primary Health Care services are provided free at the point of need for people who are registered with Hauora Hokianga. A koha or donation is welcomed from those who are able to contribute.
