How to Enrol:
- Contact Your Local Hauora Hokianga Clinic: Reach out to the receptionist at your local clinic enrolled GP practice to express interest in signing up for Health365.
- Complete the Consent Form: Fill out the required consent form, which is also available online or available from your local clinic.
- Provide Your Email Address: Ensure you have an individual personal email address, as this will be used for account setup and communication.
- Receive Confirmation Email: After processing your registration, you will be sent a confirmation email containing your username and a temporary password.
- Access the Portal: Follow the link in the email to the Health365 website, enter your credentials, and change your password to something secure and memorable.
