Health365: Your Online Health Management Portal

Health365 is a secure online service that allows you to manage your healthcare needs anytime, anywhere. Health365 provides convenient access to your medical records and a range of services to help you take control of your health. You can also be authorised to view other family members’ records on the same account (e.g. children or dependents).

Key Features:

  • Order Repeat Prescriptions: Request your regular medications to be sent directly to your preferred pharmacy.
  • Access Your Health Profile and View Medical Records: Access your medical history, immunisations, medications, and lab or test results.

How to Enrol:

  1. Contact Your Local Hauora Hokianga Clinic: Reach out to the receptionist at your local clinic enrolled GP practice to express interest in signing up for Health365.
  2. Complete the Consent Form: Fill out the required consent form, which is also available online or available from your local clinic.
  3. Provide Your Email Address: Ensure you have an individual personal email address, as this will be used for account setup and communication.
  4. Receive Confirmation Email: After processing your registration, you will be sent a confirmation email containing your username and a temporary password.
  5. Access the Portal: Follow the link in the email to the Health365 website, enter your credentials, and change your password to something secure and memorable.

Health365 prioritises the security and privacy of your health information. Your data is protected by multiple layers of encryption, and access is controlled through your unique username and password. None of your health information is stored by Health365; it serves as a secure channel connecting you with the data held by Hauora Hokianga.
