Community Nursing Service
The Hauora Hokianga Community Nursing service provides comprehensive and integrated mobile health care services in each of the remote communities surrounding the Hokianga River. It offers a single point of care for people in their community to access services ranging from preventative care to proactive support for people to live and manage their long-term medical conditions, such as diabetes, cardio-vascular, musculo-skeletal, and respiratory illnesses. The service also supports people to optimally live with disabilities in their homes and communities.
The service is provided by health care teams led by community health nurses in association with kaimanaaki tangata, clinic receptionists, specialist nurses, nurse practitioner, mental health staff, midwives, social worker, home care assistants, support staff, and visiting GPs.
