Our Community Health Services provide integrated and comprehensive care across the Hokianga, delivered through ten hauora clinics located in outlying areas throughout the rohe. General practitioner services and community nursing work together to ensure coordinated care close to home.
General Practitioner Services
Hauora Hokianga General Practitioners (GPs) bring a diverse range of specialist interests, including:
- Rural medicine
- Mental health
- Palliative care
- Emergency medicine
- Obstetrics
- Teaching
- Minor surgery
Community Health Nursing
Our Community Health Nurses practise as generalist nurses, delivering a wide range of services, including:
- Public health nursing
- Tamariki Ora | Well Child
- Long‑term condition management
- Dental health services
- Cancer screenings
- Nursing support during GP clinics
School Nurse and Diabetes Nurse Specialists work alongside the community health nursing team to provide integrated care.
Nurse Practitioners further strengthen the service by focusing on chronic condition management and cardiac rehabilitation.
Community Support and Allied Health
Kaimanaaki Tangata (KMTs | Community Health Support Workers) are part of a wider multidisciplinary team supporting clients to maintain optimum health and wellbeing. KMTs bring together professional expertise and strong community knowledge to support whānau at risk in culturally appropriate ways.
Within the KMT team, some roles are specialised in Cancer Care and Health Promotion.
