Hauora Hokianga’s Needs Assessment and Service Co-ordination (NASC) team supports people to manage disabilities and long-term health conditions, helping maximise independence and maintain quality of life within the community.
This service is available to adults aged 65 years and over, and to adults under 65 with very high support needs related to chronic conditions, who live in the Hokianga region.
